Our office is located in North Riverside IL, conveniently accessed at the corner of Harlem and Cermak with plenty of free parking.

Appointments

We understand occasional circumstances prevent you from keeping your scheduled appointment. If possible, please notify our office in advance if you are running late or unable to make your appointment. Our scheduling coordinators will do their best to re-schedule you promptly. There is a $50 no-show fee.

Insurance and Billing

We proudly accept, Blue Cross Blue Shield PPO and HMO (through Loyola and MacNeal) Aetna PPO and HMO (through Loyola and MacNeal), Cigna, Tricare, Allied Benefits, UMR, United Healthcare, Humana, Straight Medicaid/All kids, Blue Cross Community, Meridian, Molina, Aetna Better Health and County Care. We also have a reasonable cash fee schedule for patients without insurance. Keep in mind, vaccines for uninsured children are covered by the Vaccines for Children Program and provided for a minimal administration fee.

Payment Options

Payment is due at the time services are rendered, we accept cash, checks and all major credit cards.
