Thank you for choosing MP Pediatrics as your child's medical home. We've included helpful items on this page to ensure you and your child's first trip to our office is productive, stress-free and enjoyable.

If you're preparing for your child's first visit, you can do a few things to help expedite the appointment. Please provide the following information when you arrive for your visit:
- Insurance cards
- Complete immunization records
- Parent's photo ID (A parent or legal guardian must bring the child to their first appointment. If you are not the parent, please bring legal guardianship or other court documents)

If you need a school physical, WIC form, sports physical, back to school or work note, please let us know when you arrive so we can have them ready for you.
